If you are required to use a paper form, send your form through the MyHealth@Work secure fax line at 412-647-5051.

Or visit Employee Health Services Clinic conveniently located on the Pittsburgh campus at 3708 Fifth Avenue, Medical Arts Building, Suite 505.

Hours of operation are 7:00 a.m. and 3:30 p.m. Monday through Friday.
